Action Item Tracking
The practice of recording the follow-up tasks identified in a post-incident review and tracking each one to completion with an owner and due date, so lessons from an incident actually turn into fixes.
Without tracking, post-incident reviews produce good ideas that quietly never happen, and the same gaps cause the next incident. Effective tracking assigns each item an owner, a deadline, and a status, and revisits open items in later reviews.

- Logging 'enable MFA on the VPN' as an action item with an owner and a two-week deadline after an incident.
- Reviewing the open action items from last quarter's incidents at the start of a new postmortem.
- Closing an action item only after the control change is verified in production.
