Action Items
The concrete follow-up tasks that come out of a post-incident review, each describing a specific improvement, such as a configuration change, new detection, or process update, meant to reduce the chance or impact of a future incident.
Good action items are specific, assignable, and verifiable rather than vague aspirations. They are the bridge between learning what went wrong and actually changing the environment so it does not recur.

- 'Add alerting for new mailbox forwarding rules' written up after an email compromise.
- 'Document the on-call escalation path' after confusion delayed a response.
