MFA Implementation
How an organization actually deploys MFA, choice of factors, coverage of users and apps, enrollment and recovery, fallback, and protection of the MFA system itself, so MFA delivers real security rather than gaps and friction.
MFA's effectiveness depends on implementation: which factor types are used (preferring phishing-resistant where possible), whether all users and applications are covered, how enrollment and account recovery are secured (since recovery is a frequent bypass), what fallback is allowed (legacy or backup factors can be weak links), and how the MFA system is itself protected. A well-implemented MFA program closes gaps and resists attacks like MFA fatigue and SIM-swap, while a poor implementation leaves exploitable seams.

Examples
- Covering all users and applications with MFA rather than only some.
- Securing account recovery so it does not bypass MFA.
- Preferring phishing-resistant factors and limiting weak fallbacks.
