Organizational Incident Learning

How an organization, not just individuals, learns from incidents, capturing lessons, changing processes and controls, and building institutional memory, so it gets measurably better after each incident.

Organizational incident learning is the leadership-level capability of turning incidents into lasting improvement: running effective reviews, translating lessons into concrete changes (process, controls, training), tracking that those changes happen, and building institutional memory so knowledge survives staff turnover. It goes beyond individual learning sharing to ensure the organization as a whole adapts. Mature programs treat every significant incident as a driver of systemic improvement rather than a one-off to move past.
